Please find mandatory forms required for participation in RARA programs below. Please read carefully to be sure that you are able to participate in our programs upon completion of the proper paperwork. Below you will find information about our waiver, prescreening form, and daily screening form.
The RARA waiver must be "signed" upon registration. This is done automatically if you register online. If you register over the phone, you can login to your account and fill out the waiver HERE. You can submit this via email or in person at the program by contacting the proper Recreation Supervisor found in our DIRECTORY
The pre-screening form needs to be completed beginning 2 weeks prior to the start of the program. That form indicates your temperature and/or symptoms experienced over this 2 week time frame. It also indicates that you have not tested positive, nor have you been in contact with anyone that has tested positive in those two weeks. This form can be found here: Two Week Pre-Screening Form. This form can be submitted on your account at rararegistration.org, emailed to the Recreation Supervisor, or turned in on day 1 of your program to our staff. If you register with less than two weeks to go before your initial start date, please reach out to your Recreation Supervisor for guidance prior to participating.
The daily screening form must be completed before arriving on site at any RARA program. This form is for programs that meet more than once i.e. sports leagues, 6-week programs, clubs, etc. Programs that are one-time programs such as Slime Class, Movie Pizza Night, and Self Defense class do not need to fill out this form. In that case, just the two week prescreening form must be filled out. The daily screening form can be found here: Daily Screening Form and is completed online only. Proof of completion must be shown to your instructor when you get there. The daily screening can also be found as a module below.
